Job Description

Catholic Charities seeks a dedicated Intensive Case Manager III Specialist to join our team. This role is responsible for providing individual and crisis intervention case management and counseling services to program clients/families in order to ensure clients maintain housing.

About the Job
  • Implement and deliver all aspects of peer support to program participants, assisting them in achieving stabilization and short/long-term goals.
  • Work with families who have/had recurring episodes of homelessness with associated disabilities to remain housed, utilizing housing subsidies and permanent supportive housing modality.
  • Provide core housing case management for families with extenuating circumstances fitting into mainstream housing situations, requiring enhanced support around physical and mental health – acute/chronic, problem-solving, and support services.

In this role, you will assess participants' needs by providing immediate services, personal goals, and service plans. You will work with participants around self-sufficiency, maintaining independence, and preventing reoccurrence of homelessness.

Responsibilities
  • Case management referrals to community support and counseling.
  • Crisis intervention counseling and support services, referrals and linkages, client data collection, information, and referral collection of documents needed for program participation.
  • Coordination of services to address clients' immediate needs.
  • Collaboration with Program, Division, Agency staff/team, and community partners to design and implement program operations and policies.
  • Interface with other agencies and departments to ensure appropriate client compliance with Program requirements.
Requirements
  • Bachelor's degree in Social Work or related field experience with a minimum of two years in case management.
  • Sensitivity to and knowledge of homelessness and substance use disorders, and associated healthcare (physical and mental health).
  • Demonstrated knowledge/experience working with families in crisis or unstable housing situations requiring intensive services.
  • Strong knowledge of substance disorders and associated health issues.

Salary: $70,000 - $90,000 per year, depending on experience.
