Job Overview

Sisters of Charity Health System is seeking a skilled Pharmacist - Outpatient Specialist to join our team. This role will be responsible for providing high-quality patient care and services in our outpatient pharmacy.

About the Job

We are offering a competitive salary of $120,000 per year, based on experience and qualifications. The ideal candidate will have excellent communication skills, be able to work independently, and possess strong problem-solving abilities.

Responsibilities
  • Quality Patient Care:
    • Assess patients' medication needs and provide personalized recommendations.
    • Review patients' medical history and current medications to ensure safe and effective treatment.
    • Counsel patients on proper medication use, potential side effects, and interactions with other medications.
  • Medication Management:
    • Order and manage medications, including prescription fills, refills, and cancellations.
    • Monitor patients' medication adherence and adjust treatment plans as needed.
    • Collaborate with healthcare providers to optimize medication regimens.
  • Administrative Tasks:
    • Manage and maintain accurate patient records and medication profiles.
    • Process prescriptions, insurance claims, and billing information.
    • Participate in quality improvement initiatives to enhance patient care and services.
  • Education and Training:
    • Stay up-to-date on the latest developments in pharmacology, medication therapy, and patient care.
    • Attend workshops, conferences, and online training sessions to expand knowledge and skills.
    • Mentor new staff members and provide guidance on best practices.
  • Communication and Teamwork:
    • Develop strong relationships with patients, healthcare providers, and support staff to ensure seamless communication and collaboration.
    • Participate in multidisciplinary teams to discuss patient cases, share expertise, and address concerns.
    • Provide clear, concise, and compassionate communication to patients, families, and colleagues.
    Requirements
    • Education: Bachelor's degree in Pharmacy from an accredited institution.
    • License: Current license to practice pharmacy in the state of employment.
    • Experience: 1-2 years of experience in retail or outpatient pharmacy settings.
    • Certifications: Certification in Immunization and Medication Therapy Management a plus.
    • Skills: Strong communication, problem-solving, and organizational skills.
