Job Title: Registered Nurse - Field Supervisor

VNS Health is seeking a skilled Registered Nurse to join our team as a Field Supervisor in Home Health. As a key member of our clinical team, you will provide exceptional care and support to patients in their homes, ensuring they receive the highest quality care and services.

Job Summary:

The Registered Nurse - Field Supervisor will be responsible for:

  • Conducting initial and ongoing clinical visits to patients' homes to assess their needs and provide training support
  • Orienting and supervising Home Health Aides in the field
  • Documenting patient visits and care plans in the Electronic Medical Record (EMR)
  • Communicating with the clinical care team to ensure timely and accurate charting
  • Providing crisis management and emergency intervention as needed
Requirements:

To be successful in this role, you will need:

  • A current license to practice as a Registered Nurse in New York State
  • A minimum of two years of experience as an RN in a home care setting
  • Valid driver's license (may be required)
