Nurse Field Supervisor

VNS Health is seeking a skilled Registered Nurse (RN) to fill the role of Field Nurse Supervisor in our Home Health department.
About the Role:As a Field Nurse Supervisor, you will be responsible for providing clinical leadership and support to our Home Health Aides (HHAs) in the field. Your primary focus will be on delivering high-quality patient care, ensuring timely and accurate charting, and conveying significant changes in patient condition to the clinical care team.
Key Responsibilities:- Perform initial clinical and ongoing supervisory field visits to patients' homes to provide assessment, training support, in-home coordination, and crisis management.
- Orient and supervise HHAs in the field, ensuring they have the necessary skills and knowledge to provide exceptional patient care.
- Extensively document every patient visit in the EMR on a continuous basis, ensuring timely and accurate charting for clinical team reference and auditing purposes.
- Convey any significant changes in the patient's condition, emergency intervention, or care plan changes to the clinical care team.
To be successful in this role, you will need a minimum of two years of experience as an RN in a home care setting. You should possess excellent clinical skills, strong communication and leadership abilities, and a passion for delivering high-quality patient care.
