Social Services Specialist

About the Role:
Bethesda Dilworth is seeking a dedicated and compassionate Social Services Specialist to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing goal-oriented and person-centered sessions to meet the social service needs of our residents.
Key Responsibilities:
- Participate in the planning of effective individualized social service programming through written assessment, multi-disciplinary treatment participation, and completion of required documentation.
- Ensure that charting is completed and maintain resident rights through social service observation and assessment.
- Develop relationships with residents and family members, addressing concerns professionally and in a timely manner.
- Provide social service assessments upon admission and for residents who have been hospitalized.
- Collaborate with the interdisciplinary team to address the needs of residents and family members within the scope of practice.
Requirements:
- High school degree or equivalent
- Bachelor of science in Social Work preferred
- Must have knowledge of growth and development and show expertise for geriatric population
- Certification as a Social Service Designee by State preferred
- One year experience in long-term care social service department preferred
