Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Dependable Hospice and Palliative Care in Tucson, Arizona.

Estimated Salary: $60,000 - $80,000 per year

Job Responsibilities
  • Identify patient/family needs and provide supportive care according to physician orders and interdisciplinary group plans.
  • Assess patient health status, communicate with attending physicians and other healthcare professionals, and coordinate optimal care and resource utilization.
  • Maintain regular communication with nurse case managers, program directors, and medical directors.
  • Obtain data on physical, psychological, social, and spiritual factors influencing patient health status and report to assigned registered nurse case managers.
Requirements
  • Current licensure as an LPN/LVN in Arizona.
  • Able to cover some after-hours and weekends.
  • Good verbal and written communication skills, and organizational abilities.
  • Licensed driver with insured vehicle and current BLS certification.
  • Hospice experience preferred.
