RN Home Health Job Description

Covenant Health Inc. is seeking a skilled RN Home Health professional to join our team. As a RN Home Health, you will provide professional nursing care to an assigned group of homebound patients, coordinating care planning with other disciplines.

Responsibilities:
  • Review patients' records, determine home care needs, and develop care and discharge plans.
  • Perform and record cardiovascular, pulmonary, neurological, GI, skin, intake and output, and pain assessments.
  • Perform treatment procedures, including insertion of nasogastric/gastrostomy tubes, replacement of gastrostomy tubes, and providing teaching and training for patient/caregiver on care of feeding tube.
  • Administer medications, including IM and sub-q medications, and medications through IV push and IV piggyback.
  • Use and maintain equipment, such as infusion pumps, ventilators, and manual resuscitators.
  • Act as liaison between MD and patient/family, communicating orders and changes in patient needs.
  • Prepare and submit daily documentation, including contact notes, mileage, and other relevant information.
  • Follow Medicare guidelines and perform in compliance with such guidelines.
  • Provide patient/family education and counseling regarding disease process, self-care techniques, and prevention strategies.
  • Make appropriate referrals and follow admission, discharge, on-call, and death procedures as outlined by Covenant Health Inc. policy.
  • Perform basic infection control techniques and supervise LPNs and Home Health Aides providing services.
Qualifications:
  • Graduation from an accredited school of nursing.
  • One (1) year of professional experience in hospital or prior home health experience required.
  • Must have and maintain State of Tennessee RN license, CPR certification, and a valid Tennessee driver's license Class D.
