About the Role

We are seeking a highly skilled and compassionate Mental Health Case Manager to join our team at the City of Chesapeake. As a key member of our Behavioral Healthcare team, you will play a vital role in providing exceptional support and services to individuals with serious and persistent mental illnesses.

Key Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each client.
  • Provide ongoing support and guidance to clients, including community outreach, treatment compliance monitoring, and coordination of care with service providers.
  • Document all services provided in a timely and accurate manner, ensuring compliance with Medicaid and DBHDS standards.
  • Collaborate with clients, families, and service providers to ensure seamless coordination of services and advocacy on behalf of clients.
  • Assist with discharge planning for clients who have been hospitalized, linking them to community resources and support services.
Requirements
  • Bachelor's degree in Human Services (psychology, social work, special education, etc.) or a related field.
  • Minimum of two years of related experience, preferably in a mental health setting.
  • Valid driver's license and a driving record that meets City Driving Standards.
  • QMHP-A certification and knowledge of DMAS standards for case management services.
  • Ability to work flexible hours, including evenings and weekends, as needed.
Preferred Qualifications
  • Bachelor's degree and at least three years of experience providing services to mentally ill clients in the social work, counseling, psycho-social rehabilitation, or case management fields.
  • QMHP-A certification and valid driver's license with an acceptable driving record.
