Home Health Physical Therapist

We are seeking a dedicated and professional Physical Therapist to join our team at Jet Health.
About the RoleThis is a full-time, part-time, or PRN opportunity to provide coordinated care to patients of all age groups in the Parker, Castle Rock, Highlands Ranch, Lone Tree, CO area and surrounding regions. As a Physical Therapist with Jet Health, you will plan, organize, and conduct physical therapy treatment programs based on medical referrals and evaluations.
Your primary responsibilities will include:
- Developing and implementing individualized physical therapy treatment plans tailored to each patient's needs;
- Evaluating and documenting patient progress, making adjustments to treatments as necessary;
- Coordinating care with other healthcare professionals to ensure comprehensive treatment;
- Educating patients and their families on treatment plans, home exercises, and preventative care;
- Maintaining accurate and timely patient records in compliance with regulatory standards;
- Utilizing durable medical equipment as needed and providing training to patients;
- Ensuring patient safety and comfort during therapy sessions;
- Participating in continuous professional development to stay updated with the latest therapy practices;
To be successful in this role, you will require:
- A Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program;
- A current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC), if practicing in a state member of the PTLC;
- At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health environment;
- Strong interpersonal skills, proficiency in therapy skills, current CPR certification, and knowledge of durable medical equipment;
- Reliable transportation, a current Driver's License, and valid and current auto liability insurance.
The estimated salary for this position is $65,000-$80,000 per annum, depending on qualifications and experience.
