">Job Summary
">We are seeking a skilled Clinical Case Manager RN to join our team at Providence Medical Group. This is a full-time position working day shifts. As a key member of our patient centered medical home (PCMH) teams led by the physician/provider, you will provide care management and care coordination services for at-risk patients, including those with complex medical and psychosocial issues.
">Responsibilities
">- Provides care management and care coordination services for at-risk patients, including those with complex medical and psychosocial issues
- Uses population-based, chronic care management approaches and clinical protocols to support patients
- Collaborates with healthcare providers to develop individualized care plans
- Communicates effectively with patients, families, and caregivers to ensure seamless care transitions
Requirements
">To be successful as a Clinical Case Manager RN, you must have:
">- Graduation from an accredited school of nursing
- Oregon Registered Nurse License
- 3 years of Clinical experience as an RN
Preferred Qualifications
">- Bachelor's Degree in Nursing
- 2 years of Home health, skilled nursing facility and/or community health nursing
- 2 years of Managed care experience and/or utilization management experience
- 2 years of Care/Case Management experience
