About the Role

We are seeking a dedicated and compassionate MDS Coordinator to join our team at EagleCare LLC. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our residents.

Responsibilities
  • Coordinate the completion of resident assessments using RAI tools to promote individualized support and care.
  • Act as a resident advocate by managing the interdisciplinary care plan process and overseeing the management of resident assessments for case mix reimbursement.
  • Transmit MDS reports according to strict guidelines and serve as a member of Quality Improvement committees to enhance positive resident outcomes.
  • Demonstrate C.A.R.E. values to our residents, family members, customers, and staff.
Requirements
  • Graduate of an accredited School of Nursing, preferably BSN.
  • Minimum of one year in nursing management in the long-term care industry.
  • Two years professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting.
  • Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data set 2.0.
  • Knowledge of OBRA regulations and familiarity of current state reimbursement guidelines.
  • Knowledge of current laws and regulations that influence provision of care and services in nursing facilities.
