Physical Therapist Job Description

We are seeking a skilled Physical Therapist to join our team at Doctors Subacute Care. As a Physical Therapist, you will play a vital role in helping our residents achieve their rehabilitation goals.

Responsibilities:
  • Work collaboratively with the therapy team to develop and implement individualized treatment plans
  • Conduct thorough evaluations and assessments to identify residents' needs and abilities
  • Provide high-quality physical therapy services to residents, including exercises, stretches, and other interventions
  • Document residents' progress and communicate with other healthcare professionals to ensure seamless care
  • Stay up-to-date with the latest physical therapy techniques and best practices
Requirements:
  • Current New Jersey Physical Therapist License
  • Experience working in long-term care settings, preferably in a Skilled Nursing Facility
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
