We are seeking an Independent Optometrist to lease space inside our Sam's Club locations. As an Independent Optometrist, you will have the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment, instrumentation, administrative, and/or Opt-Tech support services for a fair market value.

**Responsibilities**

* Provide eye and vision care services to patients
* Establish your own days of practice and hours of operation
* Control your patient schedule and set your fee schedule
* Opt into equipment, instrumentation, administrative, and/or Opt-Tech support services for a fair market value

**Requirements**

* Doctor of Optometry (OD) from an accredited optometry school
* Valid state optometry license
* Ophthalmologist (MD or DO) from an accredited school
* Valid state ophthalmology license
* General Liability Certificate
* Professional Liability Insurance Certificate
* W-9
