Key Responsibilities
- Deliver personal care services based on assigned patient needs and respond promptly to requests.
- Communicate effectively with the Case Manager regarding any challenges in task execution.
- Employ positive communication techniques while ensuring confidentiality and respect for patients and families.
- Ensure patient safety and proper use of equipment (e.g., foot stools, side rails, oxygen).
- Provide personal care including bathing, oral hygiene, and changing bed linens as required.
- Assist patients with dressing and undressing as needed.
- Plan and prepare nutritious meals, including shopping, as assigned.
- Aid in feeding patients when necessary.
- Monitor and record vital signs as per care plans.
- Observe and care for patients' skin to prevent tissue breakdown.
- Report significant changes in patient condition to the Case Manager.
- Support ambulation and exercise as directed by healthcare professionals.
- Assist with self-administered medications in accordance with state regulations.
- Provide assistance with light laundry tasks as assigned.
- Maintain cleanliness and orderliness in the patient's living area.
- Participate in quality improvement initiatives and adhere to documentation standards.

Qualifications
- Must meet state training requirements.
- Completion of a minimum of 75 hours of classroom and supervised practical training, including 16 hours of classroom training prior to practical training.
- At least six months of experience in providing personal care.
- Minimum age of 18 years.
- Ability to read, follow instructions, and document care provided.
- Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
- Strong references from previous employers.
- Ability to work independently with minimal supervision.
- Comfort with issues surrounding death and dying.
- Calm demeanor when interacting with patients in their homes.
- Current CPR certification required.
- Excellent communication and organizational skills.
