Abode Care Partners is seeking a skilled Senior Medical Care Specialist to join our team. As a key member of our interdisciplinary care team, you will provide medical services to seniors and patients with complex needs in their homes or assisted living environments.
Responsibilities- Evaluate patient medical needs and develop comprehensive plans of care
- Treat chronic and urgent conditions, minor injuries, and other illnesses
- Collaborate with healthcare professionals to ensure seamless care coordination
- Order and interpret laboratory and diagnostic testing as needed
- Active state Nurse Practitioner license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Competitive salary: $100,000 - $120,000 per year
- Weekdays and daytime shifts only
