We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.
Key Responsibilities- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Identify patients' and families' physical, psychosocial, and environmental needs, and reassess as needed, no less than every 15 days.
- Develop and implement individualized plans of care, collaborating with patients, families, attending physicians, and other healthcare professionals.
- Document patient progress, problems, goals, interventions, and responses to care in a timely and accurate manner.
- Collaborate with the patient, family, and healthcare team to provide comprehensive care and support.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Experience working with terminally ill patients.
