Job Summary

Key Responsibilities
  • Coordinate care to assigned patients, establishing goal-directed care plans and conducting ongoing assessments of patient needs.
  • Perform on-site supervisory visits to assess patient, family, environment, and clinical caregivers, with timely follow-up documentation.
  • Take on-call duties as assigned, demonstrating a commitment to providing timely and responsive patient care.
  • Uphold and promote adherence to CHAP standards, ensuring delivery of high-quality and compliant nursing services.
  • Contribute to retention initiatives by fostering a supportive work environment and providing valuable insights to enhance caregiver satisfaction and longevity.
  • Ensure availability and proper operation of necessary equipment and supplies related to patient care.
  • Assess patients and provide direct patient care as needed.
  • Promote and manage expectations and satisfaction with internal and external customers.
  • Evaluate the quality and effectiveness of nursing services and analyze clinical data to improve patient outcomes or mitigate risk.
Requirements
  • Diploma, Associate, or Bachelor's degree in nursing from a state-accredited RN program.
  • Clinical or other work experience as per state and/or federal laws and regulations.
  • RN licensure in designated state(s) as appropriate.
  • Valid Driver's license and Acceptable MVR.
  • Preferences: RN experience and knowledge of private duty program administration.
Physical Requirements
  • Ability to speak, write, read, and understand English.
  • Ability to travel; company does not provide vehicles or transportation.
  • Occasional lifting, carrying, pushing, and pulling of up to 40 pounds.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Ability to sit and climb stairs.
  • Visual and hearing acuity.
  • Strong sense of smell and touch.
  • Ability to respond physically and mentally to emergency situations.
