About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.

Key Responsibilities
  • Evaluate patients' communication and swallowing disorders, and develop personalized treatment plans to address these issues.
  • Conduct assessments, administer diagnostic tests, and interpret results to inform treatment decisions.
  • Provide individualized therapy sessions to patients, focusing on improving communication, swallowing, and cognitive skills.
  • Collaborate with healthcare teams to ensure comprehensive care and effective communication with patients and families.
  • Document patient progress, maintain accurate records, and submit reports as required.
Requirements
  • Master's degree in Speech-Language Pathology or related field.
  • Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
  • Minimum 6 months of experience as a Speech Therapist or Speech-Language Pathologist.
  • Home Health experience preferred.
  • Current and unrestricted license to practice in the state.
  • CPR certification.
  • Excellent communication and organizational skills.
