We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities- Evaluate patients' communication and swallowing disorders, and develop personalized treatment plans to address these issues.
- Conduct assessments, administer diagnostic tests, and interpret results to inform treatment decisions.
- Provide individualized therapy sessions to patients, focusing on improving communication, swallowing, and cognitive skills.
- Collaborate with healthcare teams to ensure comprehensive care and effective communication with patients and families.
- Document patient progress, maintain accurate records, and submit reports as required.
- Master's degree in Speech-Language Pathology or related field.
- Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A) from ASHA.
- Minimum 6 months of experience as a Speech Therapist or Speech-Language Pathologist.
- Home Health experience preferred.
- Current and unrestricted license to practice in the state.
- CPR certification.
- Excellent communication and organizational skills.
