About the Role

We are seeking a highly skilled and experienced Care Management Supervisor to join our team at Hackensack Meridian Health. As a key member of our care management team, you will be responsible for overseeing the coordination of patient care across the continuum, ensuring timely and efficient care delivery and reimbursement.

Key Responsibilities
  • Facilitate collaborative management of patient care, intervening as necessary to remove barriers to care delivery and reimbursement.
  • Supervise staff with care management processes, including length of stay, throughput, patient flow, and denials and appeals follow-up.
  • Apply process improvement methodologies in evaluating team members' documentation.
  • Ensure multidisciplinary rounds and team huddles are occurring.
  • Provide direction to the multidisciplinary team as needed in difficult cases.
  • Obtain, interpret, and present metrics related to care management.
  • Attend key meetings and present information about existing case management processes.
  • Develop performance improvement plans for care managers, document performance, and provide feedback.
  • Determine areas of opportunity and suggest process improvements.
  • Follow HR policies for performance and disciplinary action.
Requirements
  • Bachelor's degree in Nursing or Master's Degree in Social Work.
  • At least 3 years of full-time experience in an acute care setting.
  • Familiarity with hospital resources, community resources, and/or resource/utilization management.
  • Care coordination, case management, or discharge planning experience.
  • Effective decision-making/problem-solving skills, demonstration of creativity in problem-solving, and influential leadership skills.
  • Excellent verbal, written, and presentation skills.
  • Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • NJ State Professional Registered Nurse License or NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
  • Basic Cardiac Life Support Certificate.
