Pact Nurse Case Manager

Job Summary:
Eliot's Adult Mental Health Division is seeking a skilled Pact Nurse Case Manager to join our team. As a key member of our community-based services, you will work closely with adults with severe and persistent mental illnesses, providing comprehensive support and care.
Key Responsibilities:
- Act as a primary case manager to 3-6 consumers, providing individualized support and guidance.
- Develop and implement personalized care plans, ensuring seamless coordination of services.
- Collaborate with multidisciplinary teams to ensure comprehensive care, including medication management and nursing interventions.
- Conduct regular assessments and updates to Comprehensive Assessments and Individualized Action Plans (IAPs) for clients on your caseload.
- Participate in supervision, staff meetings, in-services, and trainings to stay up-to-date on best practices and industry developments.
- Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed, promoting independence and community integration.
Work Schedule:
Monday-Friday, 8am-4pm, with rotating 12pm-8pm shifts, one weekend per month, rotating holiday shifts, and rotating on-call schedule.
Compensation:
RN: $77,680.00 annually, LPN: $58,073.60 annually.
