About the Role

BAYADA Home Health Care is seeking a skilled Speech Language Pathologist to join our team as a Home Health Therapeutic Care Specialist. As a key member of our multidisciplinary healthcare team, you will provide high-quality rehabilitative care and skilled nursing services to clients in their homes throughout Atlantic County.

Responsibilities
  • Conduct thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
  • Develop and implement individualized treatment plans based on client needs and physician orders.
  • Provide education and instruction to patients, family members, or caregivers on rehabilitative care and activities necessary to promote client health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
Requirements
  • A current license as a Speech Language Pathologist in New Jersey.
  • Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or have equivalent educational requirements and work experience.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
