About the Role

We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a Pharmacy Professional, you will play a critical role in providing exceptional patient care and support to our clients in long-term care settings.

Key Responsibilities
  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure timely and effective laboratory testing for high-risk conditions, collaborating with the healthcare team as necessary.
  • Provide clinical and technical support to the healthcare team, documenting all communication and interactions in the medical record.
  • Implement and maintain policies and procedures for safe medication management, procurement, storage, distribution, and disposal.
  • Stay up-to-date with federal and state regulatory requirements, ensuring compliance and maintaining accurate documentation.
  • Collaborate with facility/agency staff to provide education and support on safe medication practices.
Requirements
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
