Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our Home Health team at Northern Light Health. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers, ensuring clear understanding of care plans and progress.
  • Document patient information accurately and timely, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the state of Maine.
  • High school diploma or equivalent required.
  • Ability to lift, move, and load 30-50 pounds, with prolonged periods of standing and walking.
  • Valid driver's license and reliable transportation.
Preferred Qualifications:
  • Experience in home health or hospice care.
  • Basic Life Support (BLS) certification.
  • Ability to work independently and as part of a team.
