The Primary Care Physician (PCP) plays a pivotal role in our collaborative care model. As a value-based care provider, we prioritize quality outcomes for our patients. Our multidisciplinary care team includes physicians, advanced practice professionals, pharmacists, care coaches, nurses, behavioral health specialists, and more. This integrated approach allows our PCPs to focus on fewer patients, dedicating more time to each individual, thereby enhancing the quality of care delivered.

Required Qualifications
- Bachelor's degree
- A minimum of 5 years of relevant experience
- Licensure as required by state regulations
- Graduate of an accredited MD or DO program
- Preferred specialty in Internal Medicine
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- This position involves patient interaction and requires participation in TB screening protocols.
- A strong commitment to enhancing consumer experiences is essential.

The salary range provided reflects a good faith estimate for full-time employment. Actual compensation may vary based on geographic location and individual qualifications, with a range of $199,400 - $298,300 annually.
