Key Responsibilities:
- Develop and implement care plans in collaboration with residents, physicians, and nursing staff.
- Document resident care, including care plans, resident condition, and behavior.
- Provide functional direction and assistance to LPNs and CNAs.
- Complete new staff orientation and training as assigned.
- Direct resident care, including making rounds, answering call lights, and administering medications.
- Complete monthly and quarterly assessments and documentation.
- Attend care plan meetings and communicate effectively with residents, families, visitors, and healthcare team members.
- Participate in screening potential admissions.
Requirements:
- MA RN or LPN license.
- 1 year of long-term care experience.
