Overview

AccentCare is seeking a compassionate and skilled Registered Nurse RN to join our Home Health team as a Case Manager. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and improving health outcomes in the comfort of patients' homes.

Responsibilities
  • Assess patients' needs and develop personalized care plans to meet their unique requirements
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
