As a key member of our healthcare team, you will be responsible for assessing patient needs, developing care plans, and coordinating with healthcare providers to promote timely throughput and safe discharge.
Key Responsibilities- Assess patient needs and develop individualized care plans to promote optimal health outcomes
- Coordinate with healthcare providers to ensure seamless transitions and prevent avoidable readmissions
- Utilize national standards for case management to support medical necessity and denial prevention
- Provide education to patients, families, and caregivers on healthcare options and resources
- Ensure compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. A strong understanding of case management principles and practices is essential, as is the ability to navigate complex healthcare systems.
