Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will work closely with patients, families, and healthcare teams to provide high-quality care and support. As a Hospice RN Case Manager, you will be responsible for assessing patient needs, developing and implementing care plans, and coordinating with interdisciplinary teams to ensure seamless care delivery.

Responsibilities
  • Assess patient needs and develop individualized care plans
  • Provide direct patient care services, including wound care, medication management, and symptom control
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and chaplains, to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress
  • Manage patient records and maintain accurate documentation
  • Participate in quality improvement initiatives and contribute to the development of best practices
Requirements
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility and adaptability in a fast-paced environment
