Medical Professional for Senior Care

We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services. As a Nurse Practitioner with our organization, you will play a vital role in delivering high-quality patient care to seniors and individuals with complex needs.
About the RoleThe successful candidate will be responsible for providing evidence-based medical care to patients, including examining and treating chronic and urgent episodic illnesses, minor injuries, and other health issues. You will also be required to order and interpret laboratory and diagnostic testing as needed, collaborate with caregivers and other healthcare professionals, and maintain comprehensive patient records.

Responsibilities- Provide evidence-based medical care to patients, including examinations, treatments, and referrals as needed.
- Order and interpret laboratory and diagnostic testing to inform patient care decisions.
- Collaborate with caregivers and other healthcare professionals to deliver interdisciplinary care and maintain comprehensive patient plans.
- Maintain accurate and up-to-date patient medical records.
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Strong interpersonal communication skills – verbal and written.
- Excellent time management skills with accountability for practitioner's daily workflow.
- Proficiency in medical instruments and equipment.
- Strong knowledge base of HIPAA and OSHA regulations.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- Licensed driver with valid automobile insurance.
- Ability to engage in moderate physical activity.

Benefits- Competitive compensation: $100,000 - $120,000 per year, with total earnings potential reaching $130,000+ with our monthly incentive plan based on performance.
