Job Title: Private Duty Nurse

We are seeking a compassionate and skilled Private Duty Nurse to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning home from the hospital and providing the continuous care needed to remain at home with their loved ones.

Responsibilities:
  • Implement and manage patient care plans in collaboration with physicians and healthcare teams.
  • Conduct thorough patient assessments and coordinate care to ensure optimal outcomes.
  • Administer prescribed medications, treatments, and therapies as ordered by physicians.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with healthcare teams to ensure seamless care transitions and address patient concerns.
Requirements:
  • Current, unrestricted state license as a Licensed Nurse (RN/LPN/LVN).
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test (current within last 12 months).
  • At least six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Reliable transportation and ability to travel to patient locations.
