About the Role

We are seeking a compassionate and skilled LPN to join our hospice care team at ViaQuest. As a Hospice LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Assist RNs and physicians in performing specialized procedures and duties.
  • Administer and instruct medication administration.
  • Maintain accurate patient records and progress notes.
  • Communicate effectively with RNs regarding patient and family care needs.
Requirements
  • Graduate of an accredited nursing program.
  • LPN license.
  • Nursing or patient care experience, preferably in hospice care.
  • Valid driver's license, car insurance, and reliable transportation.
  • Ability to travel throughout the assigned service area.
