Matrix Medical Network is a leading provider of clinical services, committed to improving the quality of care for older adults and other at-risk individuals. We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in Rumford, Maine.
This PRN/Part Time position offers a unique opportunity to make a direct impact on the health and well-being of others while enjoying flexible scheduling and competitive compensation.
About the Position:
As a Matrix Provider, you will conduct comprehensive health assessments, medical histories, diagnoses, and treatments, as well as provide health education and referrals. You will work collaboratively with physicians, case managers, social workers, and other healthcare professionals to deliver high-quality care.
Responsibilities:
- Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed
- Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up
- Provide services in a variety of venues, including Home Visits, Skilled Facility Visits, and Telehealth Visits
The ideal candidate will possess a Master's Degree in Nursing or a related field, current RN and NP or PA licensure, and Board certification in a Matrix-approved specialty. Strong computer skills, excellent verbal and written communication skills, and a positive and outgoing personality are also essential.
Salary: $95.00 per completed assessment, with opportunities for additional income through mileage reimbursement and bonuses.
