Wound Care Nurse

Job Summary:
As a Wound Care Nurse at HexaQuEST Health, Inc., you will provide high-quality care to patients with wounds, fistulas, percutaneous tubes, ostomies, incontinence, and actual or potential skin breakdown. You will work collaboratively with the WOCN team to implement changes in the plan of care and serve as a resource and consultant to patients and nursing staff.
Responsibilities:
• Provides treatment based on established plans of care for patients with wounds, fistulas, percutaneous tubes, ostomies, incontinence, and actual or potential skin breakdown.
• Collaborates with WOCN to implement changes in the plan of care.
• Serves as a resource and consultant to patients and nursing staff.
• Performs assessment, planning, implementation, and evaluation of care provided.
Requirements:
• Current unencumbered Oregon RN licensure required.
• Healthcare Provider Level AHA BLS required.
• BSN preferred.
• Certification in wound/ostomy/continence care preferred.
• Experience in acute care preferred.
Working Conditions:
• Must be currently active.
• Must have a minimum of 1 year of experience in wound ostomy.
• Must work 40 hours per week.
• Must be able to work normal shifts, including days.
Special Requirements:
• Must be able to work with patients who have wounds, fistulas, percutaneous tubes, ostomies, incontinence, and actual or potential skin breakdown.
• Must be able to work collaboratively with the WOCN team.
• Must be able to serve as a resource and consultant to patients and nursing staff.
