Key Responsibilities
  • Develop and implement individualized patient care plans to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and collaborate with healthcare teams to ensure comprehensive care.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • RN license in the state of employment.
  • Graduate from an accredited school of nursing.
  • One year of RN experience and clinical competence in home health care.
  • Current driver's license and ability to spend approximately 20% of work time driving to and from patient locations.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Proficiency in electronic medical records (EMRs) and other healthcare technology systems.
