Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LPN to provide high-quality patient care in the Elizabeth area.

Job Summary:

The Private Duty Nurse LPN will be responsible for delivering and coordinating quality patient care in accordance with physician orders, while maintaining confidentiality and adhering to professional boundaries.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
  • Assess patient condition and care needs, and report changes to the supervisor and/or physician as necessary.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan to ensure effective patient care.
  • Recognize changes in patient needs and responses, and implement care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, and the Nurse Practice Act.

Requirements:

  • Must be a Licensed Practical Nurse (LPN) with a valid license to practice in the state of California.
  • Must have a strong understanding of nursing principles and practices, as well as excellent communication and interpersonal skills.
  • Must be able to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
  • Must be able to travel to patient homes, and have a valid driver's license and reliable transportation.
