Role Summary:

As a Patient Care Assistant, you will play a vital role in supporting patients with their daily activities and personal care needs. Your contributions will be essential in promoting the well-being and comfort of those under your care.

Key Responsibilities:
  • Gathering comprehensive information from healthcare providers regarding patient conditions, treatment strategies, and recommended activities.
  • Monitoring and documenting dietary intake and output, as well as reporting any significant changes to the nursing staff.
  • Taking and recording vital signs, such as blood pressure, pulse, temperature, and respiratory rate as directed.
  • Conducting patient examinations to identify any medical issues, including wounds, bruises, or abnormal symptoms.
  • Assisting patients with medication reminders and nutritional supplements.
  • Documenting observations related to patient behavior, complaints, or physical symptoms for nursing staff review.
  • Maintaining current knowledge of CNA training and adhering to facility policies and procedures.

Qualifications:

Experience:
Minimum of 1 year of experience in Skilled Long-Term Care or Transitional Care settings.

Certifications:
Current BLS certification and CNA State License are required.
