Behavioral Health Clinician

Job Summary
Easter Seals Blake Foundation is seeking a skilled Behavioral Health Clinician to join our team. As a key member of our Behavioral Health Services program, you will play a critical role in providing comprehensive coordination of behavioral health services for adult and child members.
Key Responsibilities
- Assess members' behavioral health needs and eligibility for services
- Coordinate communication between members and other providers or community agencies to ensure coordination of care
- Screen, evaluate, and schedule referred members who may be in need of behavioral health services
- Research eligibility for behavioral health services
- Develop and monitor member service plans in conjunction with clinical staff
- Assist members in securing necessary behavioral health services from EBF or other appropriate providers
- Provide rehabilitation services, including living skills training, cognitive rehabilitation, and supported employment services, as indicated in members' service plans
- Provide support services, including case management, personal assistance, family support, peer support, respite care, and transportation services, as indicated in members' service plans
- May provide individual, family, and/or group counseling based on needs as determined by the Clinical Director and EBF policies
- May conduct updates on annual assessments and develop treatment plans as determined by the Clinical Director and EBF policies
- Facilitate Child and Family Team and Adult Recovery Team meetings to review progress/barriers and determine appropriateness of services and medical necessity of services
- Provide complete documentation and reporting of member behavioral health services in accordance with agency and funding requirements
- Link members to other community resources as part of outreach services
- Arrange transportation of members to behavioral health appointments as necessary
- Participate in ongoing program planning and development
- Adhere to all EBF policies, procedures, and protocols, as well as State/RHBA regulations
- Maintain EBF productivity, timelines, and quality standards set forth by EBF
- May facilitate or participate in individual or group skills training as determined by the clinical director
