Make a Difference in Home Care

At Professional Case Management, we are seeking a skilled Senior Case Manager to join our team. As a Senior Case Manager, you will be responsible for providing direct care and case management to assigned clients.

Key Responsibilities

  • Provide direct care and case management to assigned clients
  • Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services
  • Plan, implement, and evaluate care provided
  • Participate, coordinate, and manage client care conferences as needed

Requirements

  • Graduate of a state-approved school of professional registered nursing
  • BSN preferred
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current CPR certification

What We Offer

  • Competitive hourly rate of $44.00, plus shift differentials where applicable
  • Bonus eligible
