We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team in Memphis, TN. As a PRN (as needed) opportunity, you will have the flexibility to choose your schedule and make a meaningful impact on the lives of our patients and their families.
Responsibilities- Provide safe and individualized patient care, following established plans of care and maintaining patient dignity and independence.
- Monitor patient vital statistics, report abnormal findings, and administer medications as needed.
- Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
- Ensure infection control policies are maintained, report possible communicable diseases or infections, and implement infection control procedures.
- Accurately document nursing actions, communicate with family, case manager, and physicians, and document progress and outcomes for established goals.
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals, complete incident reports, and inform case managers and examine the individual following the incident.
- Ensure all medical equipment is maintained and report faulty equipment to DME for repair.
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications.
- 1+ years of nursing experience preferred.
- Home health care experience preferred.
- Able to lift and carry items weighing up to 20 pounds.
- Able to bend, stoop, push, pull, sit, and walk for periods of time.
- Must be able to communicate both verbally and in writing.
