As integral members of our interdisciplinary team (IDT), they contribute significantly to the care planning process.

Position Overview

Reporting directly to the Home Care Supervisor, the Home Care Assistant is responsible for ensuring the safe provision of personal care services to participants in their residences, following established schedules and directives.

Key Responsibilities:

  • Deliver hands-on personal care and assistance with daily living activities (ADLs) to participants, including bathing, grooming, dressing, toileting, and safe physical transfers.
  • Support participants with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Monitor and document vital signs as per instructions.
  • Assist with light meal preparation and feeding, ensuring participants receive adequate nutrition and hydration.
  • Facilitate safe transfers in and out of homes and provide medical escort services to appointments as required.
  • Support the Rehabilitation department by implementing home exercise programs as outlined in the care plan, following appropriate training.
  • Offer translation services when qualified, to enhance communication.
