Responsibilities- Implement and monitor patient care plans, ensuring seamless coordination of care with the healthcare team.
- Assess and coordinate patient discharge planning needs, collaborating with the healthcare team to ensure a smooth transition.
- Provide direct nursing care, prioritizing patient safety and administering medications as ordered.
- Document patient care in the healthcare record, maintaining accurate and up-to-date records.
- Participate in care coordination, delegating tasks and supervising nursing staff as needed.
- Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days.
- Graduation from an accredited School of Nursing.
- Preferred: Bachelors of Science in Nursing.
