Key Responsibilities
  1. Provide support and assistance with daily living skills, including personal care, meal preparation, and household management.
  2. Create opportunities for individuals to develop their skills and foster independence.
  3. Provide services to individuals according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards, as well as Bridgewell policies and procedures.
  4. Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.
  5. Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.
  6. Assist individuals with accessing resources and recreational opportunities in the community.
  7. Administer medications according to MAP regulations, as applicable.
  8. Maintain core training certifications including FA/CPR, PABC, and MAP.
  9. Maintain additional mandated certifications and training as required by the program.
  10. Develop positive relationships with guardians, family members, and other stakeholders.
  11. Other duties as assigned.
Requirements
  1. Must have a high school diploma or GED - Associate's Degree or DSP Certification Preferred.
  2. A minimum of 1 year of experience working with people with developmental or psychiatric disabilities is preferred.
