Key Responsibilities- Medication Order Review: Interpret and evaluate medication orders prior to entry/verification, ensuring accuracy and appropriateness.
- Clinical Assessment: Clinically assess orders for dose, route, frequency, and appropriateness of medication therapy, identifying potential issues and mitigating risks.
- Medication Safety: Monitor orders for drug interactions, adverse reactions, incompatibilities, therapeutic duplication, and allergies, taking corrective action as needed.
- Order Verification: Verify the accuracy of order entry and of medication products dispensed, ensuring compliance with regulatory requirements.
- Pharmaceutical Care: Deliver optimal pharmaceutical care by participating in direct patient-care interdisciplinary rounds, advising nurses, doctors, and patients on the proper and safe use of medications.
- Narcotic Control: Assist in maintaining narcotic control and record maintenance, ensuring compliance with regulatory requirements.
- Pharmacy Operations: Oversee pharmacy technician activities, assist in developing and implementing departmental clinical and educational programs, and maintain inventory control and proper medication storage.
- Compounding: Compound extemporaneous medications, prepare IV admixtures, and inspect completed I.V. solutions for accuracy and compatibility, complying with USP 797 and USP 795 standards.
- Prescription Preparation: Prepare prescriptions in accordance with pharmacy laws and practice, ensuring accuracy and compliance.
- Education: Bachelor of Science Degree in Pharmacy or Doctorate of Pharmacy Degree from an Accredited College/University of Pharmacy.
- Licensure: Massachusetts Board of Registration in Pharmacy Licensure is required.
- Experience: 1 – 4 years clinical pharmacy experience in acute care hospital preferred.
- Certification: Board Certification (e.g. BCPS Board Certified Pharmacotherapy Specialist or other) preferred.
- Knowledge: Knowledge of medication management of adult, and older adult patients required.
