As a Home Health Registered Nurse, your responsibilities will include:

  • Providing Home-Based Care: Examine patients, do assessments, document progress, and report changes to their physician
  • Leading a Team: Work with a team of physicians and oversee LPNs, CNAs, and aides providing patient care
  • Educating Patients: Educate patients on their plan of care, prescribed medication, therapy, diet, and exercise
  • Coaching Family Members: Coach family members on the patient's plan of care, medication, and home safety

To be successful in this role, you'll need:

  • Active RN License: Hold an active Registered Nurse (RN) license in Colorado
  • Nursing Experience: Have at least 1200 hours of experience as an RN
  • Healthcare CPR/BLS Certification: Be certified in healthcare CPR/BLS
  • Home Health Care Experience: Have experience in home health care (preferred)
