Job Description

As a Home Risk Assessment Nurse, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health status, medical history, medication adherence, social environment, and other risks. This critical work provides unmatched insight into patients' overall health and well-being, allowing us to identify potential care gaps and develop targeted interventions to improve health outcomes.

You will work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to ensure seamless care coordination and delivery. Your expertise as a Nurse Practitioner will be essential in developing personalized care plans that address patients' unique needs and goals.

Responsibilities

  • Conduct comprehensive health assessments, including medical history, diagnosis, treatment, health education, and follow-up care.
  • Collaborate with multidisciplinary teams to develop and implement personalized care plans.
  • Work closely with patients, families, and caregivers to ensure accurate understanding and implementation of care plans.
  • Provide education and support to patients and caregivers on disease management, self-care, and health promotion.
  • Maintain accurate and detailed records of patient interactions, assessments, and interventions.

Requirements

  • A Master's degree in Nursing or related field; or equivalent experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A minimum of 1 year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health record software.
  • Excellent verbal and written communication skills with patients, colleagues, and other stakeholders.

This is a PRN/Part-Time opportunity with flexible scheduling options.
