JOB PURPOSE OR MISSION
Responsible for designing and executing tailored rehabilitation programs for designated patients in accordance with established physical therapy principles and practices. Engages collaboratively with a multidisciplinary treatment team. This role is performed with consideration for the specific age demographics served as outlined in the department's service scope.

JOB FUNCTIONS

ESSENTIAL JOB FUNCTIONS include, but are not limited to:

1. Conducts patient evaluations and treatments.

PERFORMANCE STANDARDS:
  • Evaluates patients, devises and implements treatment plans, regularly assesses patient progress towards goals, and documents all patient-related evaluations and treatments.
  • Creates and executes treatment programs based on evaluation outcomes, discharge objectives, and interdisciplinary team goals.
  • Provides education to patients and families, including specific exercises and motor learning strategies to enhance strength, endurance, coordination, self-care skills, sensory stimulation, mobility, and gait training. Develops home exercise programs.
  • Modifies the environment to support patient progress.
  • Documents care provided accurately.

PERFORMANCE STANDARDS:
  • Maintains patient records, billing processes, and productivity management in compliance with departmental documentation standards.
  • Encourages a collaborative team approach.

PERFORMANCE STANDARDS:
  • Collaborates effectively with team members to ensure a comprehensive interdisciplinary patient program and discharge objectives, participating in interdisciplinary team meetings and patient planning sessions.
  • Assesses and implements discharge planning activities, making appropriate recommendations regarding discharge needs, including home evaluations and equipment orders as necessary.
  • Coordinates with external vendors to procure patient-specific equipment once funding is confirmed.
  • Provides training to staff to facilitate patient progress regarding equipment, transfers, mobility, etc.
  • Promotes education and engages in departmental tasks.

PERFORMANCE STANDARDS:
  • Participates in continuing education to meet licensure requirements and stays updated on current treatment methodologies and equipment.
  • Attends mandatory meetings, functions across multiple clinical areas, and works at various locations as required.
  • Engages in departmental management tasks; supervises and/or provides training for licensed assistants, rehabilitation technicians, students, hospital staff, and volunteers.
  • Offers recommendations for process enhancements; suggests to management any equipment or supplies that may improve the Rehabilitation Program.
  • Completes additional duties and special projects as assigned.

PERFORMANCE STANDARDS:
  • Meets project deadlines consistently.

SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Current Louisiana licensure as a Physical Therapist.
Strong oral and written communication skills.
Ability to function effectively in an interdisciplinary team environment.
Excellent organizational skills.

HIPAA REQUIREMENTS:

Maintains knowledge of and complies with all relevant HIPAA regulations pertinent to the job position, including but not limited to: medical records (both paper and electronic), patient demographics, lab and radiology results, surgical or appointment schedules, patient location information, religious beliefs, public health records, quality/data related medical records, patient financial information, third-party billing, patient-related complaints, and research data.


SAFETY REQUIREMENTS:

Maintains knowledge of and adheres to all applicable safety practices relevant to the job position, including but not limited to: incident reporting, waste management, handling of sharps and linens, personal protective equipment (PPE), exposure control plans, hand hygiene, environment of care, patient identification, administering/receiving patient orders, validating, transporting, monitoring, or observing patients during procedures, and monitoring clinical alarms.
