Responsibilities:- Collect and analyze patient data to identify drug therapy needs, interactions, and potential issues.
- Develop and implement individualized medication plans to optimize patient outcomes.
- Monitor patients for desired therapeutic responses and make recommendations for modification of drug therapy as needed.
- Collaborate with healthcare professionals to ensure seamless transitions of care.
- Provide education and counseling to patients, families, and healthcare professionals on medication use and management.
- Supervise and train pharmacy technicians to ensure accurate and efficient medication preparation and dispensing.
- Stay up-to-date with the latest pharmaceutical knowledge, laws, and regulations to provide high-quality patient care.
- 3+ years of pharmacy experience, preferably in a specialty pharmacy or hospital setting.
- Knowledge of pharmaceuticals, drugs, narcotics, and related laws and regulations.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team to achieve patient-centered care.
- Experience in home care, specialty pharmacy, or hospital setting.
- Doctor of Pharmacy degree or equivalent.
- Completion of a post-graduate residency program or equivalent experience.
