As a Telephone Case Manager RN with Optum, you'll play a vital role in supporting a diverse member population with education, advocacy, and connections to resources that promote better health outcomes. With a focus on preventive care, you'll help patients navigate the healthcare system and connect them to key support services, ensuring they receive the care they need to thrive at home.
Key Responsibilities:
- Assess, plan, and implement care strategies tailored to individual patient needs, focusing on the most appropriate and least restrictive level of care in their home.
- Manage members through acute medical situations and work with chronic conditions, including testing, diagnostics, and interventions.
- Identify, initiate, and follow up on referrals for community-based programs and providers, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care.
- Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
- Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements:
- Associate's degree or higher.
- Current unrestricted Registered Nurse license in Washington DC.
- Certified in Basic Life Support.
- 1+ years of experience in post-acute care, such as long-term care.
- 1+ years of experience with using an Electronic Medical Record.
- Valid Driver's License.
- Access to reliable transportation.
- Ability to travel up to 100% to visit members in their homes within Washington DC.
Preferred Qualifications:
- 1+ years of experience working with the geriatric population.
- Experience conducting inservices.
- Field-based experience going into patients' homes.
- Experience creating care plans.
- Case Management experience.
Washington, D.C. Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors, including local labor markets, education, work experience, certifications, and more. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
