Job Summary:

We're seeking a highly motivated and compassionate individual to join our team as a Field Based Community Health Worker. In this role, you'll work closely with Medicaid members to assess their needs, develop personalized care plans, and connect them with community resources. Your expertise will help ensure that our members receive the most appropriate and least restrictive level of care.

Key Responsibilities:
  • Assess and plan care strategies that are individualized by patient and directed toward the most appropriate level of care
  • Utilize company and community-based resources to establish a safe and effective case management plan for members
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team
Requirements:
  • Bachelor's Degree in a health-related field or social work, or Active LPN license
  • 1+ years of field-based experience
  • 1+ years of experience with knowledge of community resources, culture, and values
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams
  • Valid Driver's license, current automobile insurance, and access to reliable transportation
  • Ability to travel locally, up to 100 miles round trip, and up to 50% of the time
Preferred Qualifications:
  • Community Health Worker (CHW) Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid/Medicare population
