  • Address patient and family inquiries and provide education on medication therapy and disease state management.

Patient Care Services and Operations:

  • Conduct thorough medication reviews and provide education on medication management and disease state management.
  • Provide initial and ongoing clinical medication counseling to patients and their families.
  • Collaborate with our pharmacy team to ensure accurate and timely medication dispensing and counseling.
  • Refer patients to financial counseling for copay assistance through manufacturer or charity programs.
  • Apply principles of continuous quality improvement to ensure exceptional patient care.

Requirements:

  • Kentucky and/or Indiana Board of Pharmacy license in good standing.
  • Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
  • 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
  • Experience in ambulatory clinical pharmacy practice preferred.
  • Disease state specialty experience preferred.
  • Possesses knowledge in drug information, third party claim adjudication and regulatory requirements of pharmacy practices.
